What We Do
Bellwoods Centres for Community Living provides community-based, non-medical support for people with physical disabilities.
Services are available to people aged 16 and older and are delivered in homes, housing sites, and community settings across Toronto.

Supportive Housing
Accessible apartments with personal support services that help individuals live independently.
Attendant Outreach
Personal support services delivered in a person’s home or community to help individuals manage daily living and maintain independence.
Reintegration Care Units (RCU)
Short-term transitional support for individuals returning to the community following hospitalization, illness, or injury.
Coordinated Service Delivery
Bellwoods works with healthcare providers, community agencies, and system partners to support coordinated service delivery. This collaboration helps ensure continuity and reduce service gaps during transitions or changes in care.
