How to Apply​

Learn how to access our services and begin your application.

Who is eligible?

This service may be appropriate for individuals who: 

Are 16 years of age or older
Have a physical disability
Need help with daily activities to live in the community
Live in the Toronto area
Are able or willing to learn to direct your own support services
Have a valid Ontario Health Card (OHIP)

How Do I Apply

Most Bellwoods services, including Attendant Outreach Services and Supportive Housing, require an application through the Attendant Service Application Centre (ASAC).

ASAC is part of the Centre for Independent Living in Toronto (CILT) and manages applications for multiple service providers across Toronto.

How the Process Works

Complete the application by downloading and filling the form.

Submit your completed application by email, mail, or fax.
