Supportive Housing provides accessible, rent-geared-to-income apartments with 24-hour onsite non-medical personal support.
Residents live in their own self-contained apartments and receive scheduled support based on their individual needs.
The goal of Supportive Housing is to provide reliable access to assistance with daily living while supporting independence, skill-building, and connection to community resources.
Who Is It For?
May be appropriate for individuals who:
✓ Are 16 years of age or older ✓ Have a physical disability ✓ Need accessible housing ✓ Require help with daily activities ✓ Are able to direct their own care visits ✓ Can remain safe between scheduled support visits
This program supports people to live independently by providing reliable support services.
What Kind of Support Can This Include?
Support is non-medical and is scheduled in advance. On-call support may be available when needed.
Additional services to support independence are available, including support with life skills and connecting to community services.
Personal care and hygiene
Assistance with dressing or grooming
Toileting
Eating and meal routines
Light housekeeping
Moving safely within the apartment
How Does Service Planning Work?
Clients direct their own services. Each person:
✓ Signs a Service Agreement ✓ Develops a Client Service Plan ✓ Reviews their support needs regularly
Clients must be able to manage safely between scheduled visits.
Applications for Attendant Outreach Services are coordinated through the Attendant Services Application Centre (ASAC) at the Centre for Independent Living in Toronto (CILT).
ASAC manages assessment and waitlists in partnership with service providers, including Bellwoods.
